    Liver cancer originates in the cells of the liver. Other cancers also spread to the liver, but they cannot be considered primary liver cancer. The liver's main function is to make bile juice responsible for digesting food, storing glycogen as an energy reserve, and filtering out toxins from the blood.

    It is the body's largest organ, with two lobes, both of which can be affected by cancer.

    Types of Liver Cancer

    Liver cancer can be classified into Hepatocellular carcinoma and bile duct cancer.

    • Hepatocellular carcinoma: The most common type of liver cancer is hepatocellular carcinoma. It affects the hepatic cells of the liver tissue, responsible for most of its vital functions.
    • Bile duct cancer: The Bile duct is the tube that connects the liver to the intestine to transfer the bile juice. If the cells of the bile duct are affected, then it can lead to bile duct cancer. It leads to irregular digestion and sometimes may spread to the primary liver tissue.

    Staging of Liver Cancer

    Liver cancer, once diagnosed, is categorized into one of the four stages. These four stages follow an increasing order of cancer advancement and complexity.

    • Stage 1: Cancer is confined only to the liver tissue and has not yet reached the nearby blood vessels.
    • Strage2: Either only one liver tumor is present, which is affecting the blood vessels, or more than one tumor but none more significant than 5 centimeters.
    • Stage 3: Cancer has grown larger than 5 centimeters, or it has affected nearby lymph nodes, or it has affected nearby organs.
    • Stage 4: Cancer has spread to distant organs and lymph nodes. Mostly, bones, lungs, or nearby lymph nodes can be affected.

    Signs of liver cancer can often be discrete and not very significant in the early stages. However, it is very critical to take any of the following symptoms seriously to ensure early detection and, hence, higher chances of recovery:

    • Yellowness in eyes and skin, darker urine, and pale stool can indicate liver dysfunction (Jaundice).
    • A lump or swelling is on the right side of the tummy.
    • Unexplained rapid weight loss
    • Fatigue and constantly feeling unwell.
    • Not being able to digest properly.

    Liver cancer symptoms are challenging to point out, and thus, they are rarely diagnosed at an early stage. Symptoms are similar for primary liver cancer and secondary liver cancer (a cancer that started in another organ but has metastasized to the liver).

    Identification of risk factors that contribute to liver cancer can be helpful for cancer management and prevention. If you are diagnosed with liver cancer, doctors will strictly advise you to follow a healthy lifestyle and reduce cancer growth.

    The following are some of the significant risk factors:

    • Age: Most people who are diagnosed with liver cancer are above 60 years of age. It can also affect young individuals, but such cases are rare.
    • Gender: Due to various reasons which are not yet fully understood, men are three times more likely to get liver cancer.
    • Genetic predisposition:  There is no direct link suggesting liver cancer is genetic. However, family history may increase the risk.
    • Hepatitis B virus infection: One of the leading causes of liver cancer is Hepatitis B virus infection. More than half of the liver cancer cases worldwide are linked to HBV infections.
    • Lifestyle: Lack of physical activity, alcohol consumption, and an imbalanced diet are all reasons that can increase the risk of liver cancer.

    Diagnosing liver cancer at an early stage or as soon as possible is critical to ensure full cancer recovery. Following are the tests that your doctor may order for cancer detection:

    • Alpha-fetoprotein(AFP) tumor marker test: AFP is a tumor marker—the levels of AFP in the blood rise during a liver disease or liver cancer.
    • Liver function tests: The liver releases many substances in the blood. If an abnormality is found in the liver, it leads to a difference in the levels of these substances.
    • Imaging tests: A CT scan, MRI, or ultrasound can be performed to identify the tumor areas in the liver. Your doctor may suggest any of these methods or a combination of these.
    • Biopsy: A most definitive way of diagnosing most kinds of cancer is a biopsy. It involves extracting a small section of the suspected tissue and examining it under the microscope. The tissue sample is also used for various histopathological studies, giving a clear picture of the presence and stage of cancer.
    • Surgery: Surgery is a widely employed treatment option in case of liver cancer. A partial hepatectomy(removal of a part of the liver) may be performed to remove the cancerous and some of the healthy tissue parts.

      Treating liver cancer by surgery is popular due to the regenerative capacity of the liver, i.e., the liver tissue can regrow to its standard size.

      However, surgery can be a bit complex in the advanced stages of liver cancer when the cancer has spread to various parts of the liver. In this case, surgery can still be performed, but full cancer recovery is doubtful.

    • Liver transplant: A liver transplant can be a treatment option if a healthy donor is available. A liver transplant can enhance the quality of life of a cancer patient and give them more years to live.

      Transkantations are complex procedures, both surgically and legally. Finding a compatible donor in time is daunting; hence, not all can benefit from a transplant.

    • Ablation therapy: Cancer tissue can be destroyed by various ablation therapies. Radiofrequency ablation, microwave therapy, cryoablation, and electroporation therapy are widely used options.
    • Targeted therapy: It uses a potent drug in cancer cell killing. Some of the targeted therapies include bevacizumab, lenvatinib, ramucirumab, etc.
    • Immunotherapy: It involves engineering the immune body cells to effectively kill the cancer cells naturally. Some of the immunotherapies are durvalumab, nivolumab, atezolizumab, etc.
    • Chemotherapy: Chemotherapy involves using highly potent drugs that effectively kill cancer cells. Traditional chemotherapy is not so helpful in treating liver cancer. Doctors suggest alternate chemotherapy called the Hepatic artery infusion(HAI). HAI involves delivering the drugs directly to the hepatic artery, which directly delivers them to the liver.

    Ablation therapy and chemotherapy are harsh procedures. Healthy cells surrounding the tissue(in case of ablation therapies) and even those distant from the tumor(in case of chemotherapy) may be affected. Hair loss, fatigue, loss of appetite, and feeling weak are all side effects of these therapies.

    Targeted immunotherapies are the newest treatment options for various kinds of cancers. These therapies can work miracles for some patients but may not work for all.
