    Lung cancer originates in the Lungs, which are crucial for oxygen exchange in the body. It usually affects the cells lining the air canal of the lungs but may spread to other parts of the body if not treated early.

    Lung cancer can affect both smokers and nonsmokers. Environmental conditions like air pollution and occupational hazards also play an essential role in lung cancer development.

    Types of Lung Cancer

    Lung cancer is mainly of two types:

    • Non-small cell lung cancer(NSCLC): This type accounts for 80-85% of total lung cancer cases, making it the most common type. It includes adenocarcinoma, squamous cell carcinoma, and large-cell carcinoma.
    • Small-cell Lung carcinoma(SCLC): It accounts for 10-15% of the total lung cancer cases. However, it is quick to spread and can affect other organs.

    Stages of Lung Cancer

    Lung cancer is staged from I to IV based on tumor size, affected lymph nodes(small nodes to filter out pathogens), and metastasis(spreading of cancer).

    Non small-cell lung carcinoma(NSCLC) stages:

    • Stage I: Cancer is confined to the top lining of the lungs and hasn’t spread to lymph nodes.
    • Stage II: Cancer has metastasized to nearby lymph nodes but not to distant organs. The tumor size is more significant than stage I.
    • Stage III: Cancer has spread to the lymph nodes in the mediastinum (area in the chest between both lungs)
    • Stage IV: Cancer has spread to other body parts

    Small-cell lung carcinoma(SCLC) stages:

    • Limited stage: Cancer has spread to only one lung with or without spreading in lymph nodes of the mediastinum.
    • Extensive stage: Cancer has metastasized to the second lung or other organs.

    Lung cancer symptoms are often misdiagnosed as flu, which makes regular screening very essential for high-risk individuals.

    The earliest signs of lung cancer may include shortness of breath or a worsening cough.

    Common symptoms are:

    • Coughing that doesn’t go away or stays longer than usual.
    • Frequent chest pain.
    • Experiencing shortness of breath even after light exercises(can also be a symptom of various other diseases).
    • Wheezing (whistling sound from blocked lungs).
    • Unexplained rapid weight loss.

    Advanced-stage lung cancer symptoms may include hemoptysis(coughing blood) and worsening of all the common symptoms.

    • Gender Factor: Various studies have observed that, independent of smoking, women are three times more likely to get lung cancer than men. Scientists believe this gender bias is due to estrogen (a hormone found in women abundantly,) which is involved in the pathogenesis of lung cancer.
    • Age Factor: Older people are more likely to get diagnosed with lung cancer. According to a recent survey by the National Cancer Institute (NCI), the median age (this means half of the cases are below this age and the other half are above this age) for lung cancer diagnosis is 71 years.
    • Genetic Predisposition: While genes responsible for lung cancer development are still not thoroughly studied, scientists are now sure of their involvement. A study published in Nature Journal identified 21 genes that showed significant associations with lung cancer susceptibility.     • Environmental Risks: Living in an area with a high Air Quality Index(AQI) has increased the risk of lung cancer.

    • Molecular testing: Genes like KRAS, ALK, ROS1, RET, BRAF, RET, BRAF MET, HER2, and NETK can be tested for abnormalities. Identifying the specific genes involved can help doctors prescribe particular medications.

    • Surgery: Early-stage NSCLC is often treated with surgery. Segmental resection, Lobectomy, Pneumonectomy, and sleeve resection are some surgeries that can be performed on the patient (selection depends on the tumor, size, location, etc.).  A recent study found that patients who undergo surgical resection have 5-year and 10-year survival rates of 71 and 53%, respectively. However, not all patients are good candidates for surgery.
    • Radiation therapy: It is used to kill cancer cells with the help of high-energy radiation beams. Radiation therapy is usually given by non-invasive devices that deliver the radiation without any pain in a few minutes. Radiation can sometimes cause harm to many of the healthy cells. Newer methods like proton therapy can be used for targeted radiation delivery that poses minimum harm to healthy cells.
    • Chemotherapy: It involves the administration of a specific combination of drugs into the body by IV infusion. These drugs can kill cancer cells in the body and stop their growth. Chemotherapy is often given before or after radiation therapy. Side effects of chemotherapy include hair loss, fatigue, anemia, etc.
    • Immune therapies: It uses various drugs and vaccines to enhance the body’s natural immunity against cancer cells. Some immunotherapy drugs, like pembrolizumab, atezolizumab, and nivolumab, have been approved as treatment options for certain NSCLC.
